Insurance Information
Wake County Public School System furnishes an Interscholastic Athletic Insurance Policy that provides limited benefits for all high school students in the system who participate in high school sponsored and supervised interscholastic athletic activities. More information is included in Insurance Schedule of Benefits, How to File a Claim Form, and an Accident Claim Form.

NCAA Eligibility Center
The NCAA has established a central eligibility center to certify athletic eligibility to Division I and Division II institutions. Students, who intend to participate with or without a scholarship as a freshman in college, must register with and be certified as eligible by the NCAA Initial-Eligibility Clearinghouse.

Sports Medicine Information

Authorization to Use a Prescribed Appliance in an Athletic Contest - This form is required by the NCHSAA if using a prescribed appliance.
